Professional Experience:
Dr Sandhya Mujumdar is a clinician administrator and has held administrative positions in NHG Health, NUH, AIC and other Singapore hospitals for over three decades. She has demonstrated effective leadership in healthcare in various practice settings, such as acute & primary care and other step-down care facilities.

She is an accomplished, global healthcare executive with over 30 years of progressive experience in creating clinical excellence through quality improvement programmes including IHI’s largescale collaborative programmes, lean systems thinking and applying human factors; international accreditation, clinical audits and patient safety. She has consistently worked to develop quality, safety and clinical governance framework along with clinical governance policies & procedures. She has wide experience in developing and implementing clinical performance management systems & dashboards and developing quality audit systems. She consistently demonstrates skills in investigating sentinel events (RCA using HFACS system) and any undesirable situations including issues arising from disruptive behavior of healthcare staff. Her other strengths are in risk management and FMEA.

She is proficient in developing quality and patient safety related course content and curriculums Her teaching strength resides in teaching risk management, patient safety, root cause analysis (RCA), healthcare failure mode and effects analysis (HFMEA), quality improvement essentials & tools, project management, collaborative QI projects, clinical pathways, performance management & transforming data for quality improvement, JCI standards, and tracer methodology.

At the national level, she actively contributed to developing National Curriculum Structure for quality and patient safety. She was instrumental in codesigning interprofessional (IPE) patient safety training for NUS undergraduate students from medical, nursing, pharmacy and dental faculty

Her executive leadership experience is wide as she has provided consulting services globally, including; Malaysia, Indonesia, Brunei, Thailand, Vietnam, East Europe, India and Sri Lanka. The consulting expertise focuses primarily on supporting organizations to develop more effective governance and leadership for quality and patient safety processes; enhanced strategic planning and deployment systems; applying high-reliability and operational excellence principles and tools to ensure more repeatable and improved organizational outcomes including client experience, staff experience and engagement and value to patients.

Hospital and Health System Consultation Include:

  • As a former JCI Consultant, conducted numerous accreditation mock surveys to assess readiness to the JCI standards for Hospitals, and Ambulatory Care facilities. Supported organizations to close gaps through consultation and educational solutions and implementing systems to support future ongoing continuous survey readiness.
  • Provided consultation to healthcare organisations of different settings on setting up clinical governance framework.
  • Provided consultations and coaching to NHG Health and AIC teams in conducting high quality RCAs for adverse events and also assessing the quality of RCA reports and recommendations
  • Coaching clinics for QI projects and guiding improvement teams to spread and scale projects across larger segments.
