For over 13 years, Dr. Sahana Sharan has worked regionally and nationally at Payer and Provider organizations to innovate, build, scale and optimize value-based care and population health programs.
She recognizes that for organizations to truly succeed in the value-based care model, it is important to create financial sustainability. As such, she has successfully deployed levers in value-based care that have maximized both revenue growth and cost savings thereby promoting financial sustainability of VBC model.
Her work spans all payer types (Medicare, Medicaid, Commercial and Exchange), and alternate payment models – shared savings to shared risk and bundled payment programs.
Dr. Sahana served as the Vice President of Value-Based Systems of Care at United Healthcare where she oversaw the clinical transformation and provider partnerships with over 130 health systems and provider groups across 15 states and all lines of business.
Prior to that, she served as the National Director for Value-Based Care at Tenet Healthcare where she was responsible for the enterprise’s VBC and Population Health Programs spanning 18 markets nationally.
And in number of different roles at Henry Ford Health, she helped to build and scale population health programs and bundle payment for direct to employer model.
In late 2022, Dr. Sahana founded Elate Health Partners, a Value-Based Care advisory and consulting practice. She helps organizations strategize and operationalize the levers of value-based care to significantly improve their financial performance, achieve ROI on their investments, demonstrate quality results and grow.
Her clients include Provider and Payer organizations, Top 10 Consulting Firms and Start-Ups.
Value-based care results demonstrated:
- At United Healthcare, she led the design and implementation of population health strategies to achieve $250 Million in annual cost savings and achieved 4.5 STAR rating for the regional MA business; Improved NPS scores for ACO groups and health plan market leadership from ‘good’ to ‘great’ category.
- At Tenet Healthcare, she helped build and scale population health infrastructure to attain over $100 million in annual cost savings across different value-based arrangements.
- At various provider organizations, she helped define pricing strategy for bundled payment episodes, patient care pathways, established high-performing post-acute care networks and negotiated several successful health plan contracts.
